# How to read a CLIA record — certificate types, dates, test volumes — clia-laboratory-records

> What a CLIA certificate type means, what the dates are, why the test volumes do not add up, and what the file cannot tell you.

## How to read these records

### The CLIA number

Ten characters: two digits for the state, the letter D, seven digits. One number per laboratory site, unless a
single certificate covers several sites — the record then says how many.

### Active or terminated

“Active” is the status in the CMS file for Q2 2026, whose latest record is dated Jun 29, 2026. For an
active laboratory the date shown is the expiration of the current certificate; for a terminated one it is the date the
certificate ended, with the reason CMS recorded (closure, nonpayment of fees, no longer testing, and so on).

- 1,410 laboratories are marked active with an expiration date already past in the file: their record says
so, and no page lists them. A renewal may simply not be in the file yet.
- 11,755 active laboratories carry only a person's name — typically a physician's office. They are found by
CLIA number or by name, and no page lists them.

### The certificate

Five types, from the waiver (simple tests only) to compliance and accreditation (moderate and high complexity
testing). [What each one allows](/certificate.md). The certificate type says what a laboratory may do; it is
not a grade.

### Test volumes

The file carries up to five annual volumes, from different forms: the volume the laboratory reported on its CMS-116
application, the volume verified at the state survey, and estimates of waived and provider-performed microscopy tests.
They are shown side by side, never added up, and never used to rank anyone.

### Accreditation

A laboratory reports the organizations that accredit it; the file then says whether that was confirmed. “Not
confirmed” means the match is not in the file — not that the laboratory lost its accreditation.

### What is not here

The laboratory director's name, phone and fax numbers, inspection findings, proficiency testing results, and the
list of tests a laboratory performs. Records issued after Jun 29, 2026.

**For owners and managers: does your
organization have 30 to 200 employees?**

AI labs pay companies to license their internal workflows (SOPs,
documentation, support tickets), anonymized and under contract — never patient data. You keep ownership and pay
nothing.
