How to read these records

The CLIA number

Ten characters: two digits for the state, the letter D, seven digits. One number per laboratory site, unless a single certificate covers several sites — the record then says how many.

Active or terminated

“Active” is the status in the CMS file for Q2 2026, whose latest record is dated Jun 29, 2026. For an active laboratory the date shown is the expiration of the current certificate; for a terminated one it is the date the certificate ended, with the reason CMS recorded (closure, nonpayment of fees, no longer testing, and so on).

The certificate

Five types, from the waiver (simple tests only) to compliance and accreditation (moderate and high complexity testing). What each one allows. The certificate type says what a laboratory may do; it is not a grade.

Test volumes

The file carries up to five annual volumes, from different forms: the volume the laboratory reported on its CMS-116 application, the volume verified at the state survey, and estimates of waived and provider-performed microscopy tests. They are shown side by side, never added up, and never used to rank anyone.

Accreditation

A laboratory reports the organizations that accredit it; the file then says whether that was confirmed. “Not confirmed” means the match is not in the file — not that the laboratory lost its accreditation.

What is not here

The laboratory director's name, phone and fax numbers, inspection findings, proficiency testing results, and the list of tests a laboratory performs. Records issued after Jun 29, 2026.
